How the Model Works:
1. Referral & Intake
Hospitals and partners submit referrals for individuals who are medically stable but lack appropriate discharge placement.
2. Structured Review
Each referral is assessed for: medical stability, support needs, program fit
Clinical advisory input may be utilized when appropriate.
3. Stabilization Environment
Accepted Individuals are placed into structured environments designed to supportrecovery, which may include private or accommodations to promote comfort and stability.
4. Ongoing Coordination
We support communication and alignment with care teams and partners throughout the stabilization period.
5. Transition Planning
Individuals are prepared for:
long-term housing, continued care,
next-step placement
